TAY Case Manager

POSITION:  TAY Case Manager

REPORTS TO:  Program Manager

LOCATION:  San Bernardino City, CA

CLIENT SERVICES

  • Meeting with clients weekly, creating and maintaining individual client goals.
  • Connect client with needed and available community resources. Follow-up with client and agencies to document use/success of referral.
  • Ensuring appropriate emergency shelter resources are made available to client such as, counseling, support groups, and classes.
  • Address client issues and concerns, working as a liaison with shelter manager.
  • Ensure that all documents submitted on behalf of a client are valid.
  • Conduct intake interviews and asses each client upon entry to the program for needs, strengths and independent living skills. Identify the level of need for medical/dental, social, educational needs and support and coordinate supports and resources with System Navigator
  • Provide clients with referrals and resources, then assists clients in application and follow up, while collaborating with System Navigator
  • Create linkage between TAY Transitional Living youth and resources and groups at the Drop in Centers.
  • Responds to security concerns appropriately, and documents observations and actions taken.
  • Input client data into Apricot and HMIS
  • Provide all required documentation in a timely manner, which may include client follow-up, outcome evaluation, and evaluation.
  • Identifies repairs and/or household needs and coordinates with maintenance or others to ensure needs are met, and is not above motivating and teaching youth, and participating in, indoor and outdoor upkeep and cleaning
  • Completes and maintains case notes and documents service statistics.
  • Facilitates safety of residents by doing periodic inspections, teaching fire safety, and conducting regular fire drills.
  • Attends community resource meetings to learn potential resources.
  • Other tasks assigned by the manager.

MINIMUM REQUIREMENTS

  • Knowledge of community resources and counseling/social work practices with high-risk population, as well as an understanding of Trauma-Informed and Harm Reduction approaches with residents.
  • Excellent documentation skills.
  • Excellent written and verbal communication skills, ability to establish rapport.

  • Must be able to pass CPR and First Aid Training

TAY Case Manager – San Bernardino

Position Overview:

The TAY Case Manager is to provide case management services to formerly homeless LGBTQ youth who are in our 90 day shelter. Health Navigator

POSITION: Health Navigator
REPORTS TO: Chief Operating Officer

Social Media Coordinator

REPORTS TO: Supervising Manager

HOURS: This is a full time, temporary position, which may include some weekend and evening shifts. This position starts 3/7/22 or until filled and ends May 31st, 2022.

SALARY: $20.00 per hour.

BASIC DUTIES: Responsible to develop social media content that engages, educates, and stimulates positive comments regarding the COVID 19 vaccine. Additional duties include meticulous and accurate tracking of outcomes as well as participating in or organizing in person events.

DIRECT DUTIES:

  • Manage the implementation of virtual flyers and other COVID-19 related informational resources through all social media platforms. Including, brochures, announcements, etc.
  • Manage and track each person’s individual response to the information being posted.

Rental Assistance Advocate – TAY

AREAS OF RESPONSIBILITY

DIRECT DUTIES

  • Must be able and willing to provide emergency hot-line counseling and suicide prevention support
  • Supervise clients/volunteers in a positive, caring manner to make sure staff work together as a cohesive group
  • Maintain confidentiality at all times regarding client and staff files and information about clients and/or staff
  • Supervise, monitor and mentor youth clientele in a professional manner, in accordance with licensing (refer to Title 22) and policies of the agency; client safety is a priority and clients must be accounted for all times
  • Facilitate groups with clients assigned by Shelter Manager
  • Answer phones, take messages/make calls: follow up, referrals, parents / guardians

MINIMUM REQUIREMENTS

  • High school diploma or equivalent.
  • Must have a genuine respect for and interest in youth and their families.
  • Ability to put into practice all aspects of Positive Youth Development (training will be provided) and demonstrate respect for client’s rights and safety.
  • Former Foster Youth is a plus, but must be at least 21 years old.
  • Must attend orientation
  • Ability to complete the required training and incorporate the techniques taught
  • Must be able to pass CPR and First Aid Training
  • Pass a physical and a TB Test.
  • Pass a background check, and be open and honest about any convictions
  • Excellent computer skills and knowledge of Microsoft Office.
  • Must have a valid driver’s license and vehicle with a valid registration and insurance, have a good driving record, and be willing to travel between offices and units during the workday.
  • Have received at least the first dose of Covid-19 vaccination at employment time.
