TAY Case Manager – San Bernardino
Direct Service Staff:
Positions are available for a LGBTQ TAY (Transition Age Youth 18-25) Case Manager in San Bernardino Ca. Current opening includes full-time positions only. Weekend and some evening shifts will be required.
Position Overview:
The TAY Case Manager is to provide case management services to formerly homeless LGBTQ youth who are in our 90 day shelter. Under the supervision of the Program Manager, the Case Manager is responsible for developing and maintaining case management services. Duties include: Completing intake assessments, weekly case management development, progress monitoring, finding housing resources and other referrals. Meeting with clients weekly, creating and maintaining individual client goals. Connect client with needed and available community resources. Follow-up with client and agencies to document use/success of referral. Ensuring shelter resources are made available to client such as, counseling, support groups, and classes. Ensure that all documents submitted on behalf of a client are valid. The person will also be responsible to ensure there is plenty of food in the house and all basic needs are being met.
Qualifications:
Knowledge of community resources and counseling/social work practices with homeless LGBTQ youth. Excellent documentation skills. Excellent written and verbal communication skills, ability to establish rapport. Ability to motivate others towards achieving goals. Pass a back-ground check. Ability to complete the required training and incorporate the techniques taught. Must have valid Driver License and Auto Insurance.
Compensation:
Salary range $18.00-$20.00 hour based on education and experience. Paid vacation and sick time, paid holidays, and paid health insurance for full time positions.
